Prosedur Pengajuan Pemodalan


1. Pemohon Mengajukan Proposal
2. Penelitian
3. Verifikasi Faktual (Survey)
4. Penetapan Proposal yang memenuhi Syarat
5. Pencairan Dana

Persyaratan Pengajuan Proposal Koperasi :


1. Koperasi yang disyahkan berdasarkan Undang-undang Nomor 17 Tahun 2012 dan mempunyai usaha baik simpan pinjam, penyaluran bahan-bahan kebutuhan pokok/jasa anggota yang sudah berjalan
2. Mempunyai anggota aktif, usaha yang layak dan telah melaksanakan Rapat Anggota Tahunan
3. Pengurus sanggup bertanggung jawab atas penggunaan dan pengembalian yang dipakai usaha koperasi dengan membuat surat pernyataan bermaterai Rp. 6.000,-
4. Mengajukan Proposal ke Bupati melalui Dinas dengan melampiri :

  • Foto Copy Badan Hukum Koperasi
  • Foto Copy jaminan Sertifikat Tanah
  • Foto Copy Surat Ijin Usaha Perdagangan (SIUP), Tanda Daftar Perusahaan (TDP), Nomor Pokok Wajib Pajak (NPWP)
  • Pas Foto berukuran 3 X 4 terbaru 3 lembar (Ketua Koperas)
  • Susunan Pengurus dan Pengawas yang masih berlaku dan diketahui Dinas
  • Foto Copy KTP Pengurus
  • Surat Keterangan Domisili
  • Laporan Neraca dan Rugi Laba 2 Tahun terakhir
  • Pakta Integritas
  • Proposal dibuat rangkap 4 (empat)

Persyaratan Pengajuan Proposal Usaha, Kecil dan Menengah :


1. Surat Permohonan yang ditujukan kepada Bupati Cq Kepala Dinas Koperasi dan UKM Kab. Bojonegoro
2. Foto Copy Surat Ijin Usaha Perdagangan (SIUP), Tanda Terdaftar Perusahaan (TDP), dan Nomor Pokok Wajib Pajak (NPWP)
3. Foto Copy Kartu Tanda Penduduk (KTP) dan Kartu Susunan Keluarga (KSK) yang masih berlaku bagi yang telah berkeluarga (Suami/istri) 2 lbr
4. Pas Photo ukuran 4X6 terbaru 2 lembar
5. Foto Copy bukti Anggunan/Jaminan yang sah berupa Sertifikat Tanah Hak milik dan atau BPKB Roda 4 yang nilainya lebih besar dari nominal perolehan pinjaman.
6. Apabila Anggunan/sertifikat/BPKB atas nama orang lain, diwajibkan membuat surat pernyataan pemilik yang diketahui Kepala Desa/ Kelurahan setempat
7. Surat Taksiran harga tanah yang diketahui Kepala Desa/Kelurahan.
8. Foto copy Surat Nikah pemohon dan pemilik jaminan atas nama orang lain disertakan foto 4X6 terbaru 3 lembar
9. Neraca Perdagangan 2 tahun terakhir.
10. Foto Copy SPPT/Pajak Tahunan yang berlaku
11. Foto Copy SPPT / Pajak Pajak Tahunan yang berlaku.
12. Foto Copy Kartu Pajak
13. Proposal dibuat rangkap 4 (empat)

Persyaratan Pengajuan Proposal Usaha Mikro :


1. Surat Permohonan yang ditujukan kepada Bupati Cq Kepala Dinas Koperasi, Usaha Kecil dan Menengah Kab. Bojonegoro
2. Foto Copy Kartu Tanda Penduduk (KTP) suami istri yang masih berlaku
3. Foto Copy Kartu Keluarga
4. Pas Photo ukuran 4 X 6 terbaru 2 lembar
5. Foto Copy BPKB Kendaraan Roda 2 Minimal Tahun 2009
6. Proposal dibuat rangkap 4 (empat)
